We are seeking a Community Health Worker to help people in our communities live healthier, fuller lives. This role works alongside our providers to support patients through health coaching, nutrition and fitness education, home visits, screenings, resource connections, and practical support that helps people overcome barriers to better health.
Some days, you may help a patient navigate community resources. Other days you may encourage someone working toward a healthier lifestyle, deliver a Meals on Wheels route, or help track progress as lives begin to change for the better.
This is more than a healthcare position. It is a ministry of caring, encouragement, and neighbor helping neighbor.
